Please bring as many of the following documents with you as is possible!
1. Income verification items for each working adult - 2017 tax statement OR 2017 W2 forms OR 2 consecutive most recent pay stubs AND proof of any other income sources such as child support, social security, etc
2. Official Birth Certificate - verifies child's age eligibility
3. Proof of Residency - driver's license or utility bill with name and address in MI
