- Original physical copy of Birth Certificate (not acceptable - emailed, faxed, or copies)
- Proof of residency - Current electric, gas, water, sewer bill or current lease showing address and signatures
- Current Immunization record
- School of Choice Form - Appropriate form for current residence. See information at this link
Please print enrollment packet single sided, and return completed forms with documents to the Central Elementary main office
